Organize medication, health, communication, emergency-contact, and care-team information so it can be handed to medical professionals or first responders when speaking is difficult or details are hard to recall.
Write by hand
Print the blank cards at Actual Size or 100%, then complete them with a dark permanent pen.
Complete the cards yourself, ask a trusted person for help, or optionally use an AI tool you already understand and trust. AI is never required.
Always verify medical information using medication labels, patient portals, clinicians, or other reliable records.
